All BLC players must complete and submit this form along with a US Lacrosse form and the '09 regsitration fee.

All players, High School & Middle School, must complete and submit this registration form for the 2009 season.  You must also complete a US Lacrosse Form.
US Lacrosse Registration Form 
EVERY BLC players MUST complete and submit a US Lax Registration Form annually.  There is no addtional charge as the US Lax fee is included in your BLC fee (no seperate check required.
BLC Code of Conduct Form (PDF Format) 
This is the BLC Code of Conduct/Drug & Alcohol policy form in a PDF format.  Each high school player must sign and submit this form along with their registratino form and US Lacrosse form (3 forms) plus their fee.
BLC Code of Conduct Form (Word Format) 
All high school players must complete and submit the BLC Code of Conduct/Drug & Alchol Policy. This form must be submitted along with the Registration form and US Lacrosse form (3 forms) plus thier fee.
